Learner Template Forms

Learner Template Forms can be used to record multiple instances of the same form for your learners. They also allow signatures to be captured every time a new instance of the form is completed. For more information about Learner Template Forms, please click here.

To complete a Learner Template Form on a portfolio, an instance will need to be created.

Follow the steps below but create the Form instance:

  1. Navigate to the learner’s portfolio
  2. Click the Forms tab in the "Information & Options" section
  3. Click the form Title
  4. Click Create New Instance
  5. Change the name (optional)
  6. Click Create

Completing the Form

Please be aware that the form can only be created/completed by the first signature required on the form. If the option to create a new instance isn't visible, it's likely that the form has been set up to be completed by a different user role - contact a Centre Manager to confirm this.
